Scar Tissue

Here is a failure I did not have a name for until this week. AI documents what went wrong and how we fixed it, even when the thing that went wrong should never have happened. Every agent that reads it afterward inherits the wound instead of the way. We started calling it scar tissue. The fix is subtraction: take out the thing that had to be corrected, rather than adding a note explaining the correction.
